ORDER FORM - Fill out one Order Form per collage; selecting collage style, monogram style, frame color, etc.

PACKING TAPE & RETURN - included in each box is everything needed to easily pack and ship the box back to us! Reseal the box using the Maya's Crayon Paper Tape (dampen to adhere) and ship your box back to us.  

COLLAGE ENVELOPE - Include up to 16 pieces in each collage, separate those to be included in the envelope, and leave those to be digitized only, loose in the box. Feel free to organize and make notes for any special instructions and pieces!
